Expo 2024 – Care Coordinator

Care co-ordinators help to co-ordinate and navigate care across the health and care system, helping people make the right connections, with the right teams at the right time. They can support people to become more active in their own health and care and are skilled in assessing people’s changing needs. Care co-ordinators are effective in bringing together multidisciplinary teams to support people’s complex health and care needs.

They can be an effective intervention in supporting people to stay well particularly those with long term conditions, multiple long-term conditions, and people living with or at risk of frailty.

Statement from Katie, Care Coordinator at Regal Chambers:

“I have always wanted to help people I’ve been a domiciliary carer, a hospital volunteer and a parent carer for my own neurodiverse children. This role has allowed me to provide support and care to the most vulnerable people which is my passion but yet provides me with the flexibility to work around my family commitments. 
 
The best part about being a care coordinator is the amazing team I am part of and the support we all give each other. Any problems arise someone will have your back and advise on new places to signpost to or support people. 
 
Mainly I focus on our learning disability patients and try to put adjustments in place to allow them to attend their annual reviews and any other appointments they may need. Either by creating a quiet, calm area for them to wait in or just organising all their appointments to be on the same day so they don’t have to come to the surgery as often. There is no one size fits all approach and my role enables patients to be given the extra time and support they need.”
